Tillery Compassionate Care is the first in North Carolina

Certified as Age-Friendly Care at Home

June 4, 2024 (ALBEMARLE, NC) – Tillery Compassionate Care has been officially named an Age-Friendly Care at Home organization by Community Health Accreditation Partner, Inc., (CHAP). Tillery Compassionate Care is the first hospice in North Carolina to achieve Age-Friendly Care at Home certification.  

CHAP’s Age-Friendly Care at Home certification demonstrates Tillery Compassionate Care’s value to its community, patients and referral partners, showcasing its commitment to excellence.  “The 4Ms framework is evidence-based care and addresses the unique needs of older adults which leads to higher quality of life,” said Teresa Harbour, Chief Operating Officer with CHAP.

Age-Friendly Care Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States that follows an essential set of best practices, causes no harm and aligns with what matters to the older adult and their family caregivers.  Age-Friendly Care is health care that addresses a patient’s unique needs and wants, aiming to help the patient enjoy a better quality of life.  It is safe and based on what research shows are the most important things to pay attention to as we get older- the 4Ms: what Matters, Medication, Mentation, and Mobility.

Patient Care Manager & Director of Nursing of Tillery Compassionate Care, Renee Morton views Age-Friendly Care as an approach to care that is “focused specifically on the needs of people as they age in their home setting,” she says.  “Our staff was already focusing on what’s most important to our patients along with addressing medications, mentation, and mobility in each visit.  Achieving this certification was another way for us to validate our care to our patients.”

During visits with our patients, our staff focuses on the following essential aspects of patient care, integrating the 4Ms framework into delivery of care. 

  • What Matters: Understand patient’s goals. It’s important to hear patient and family concerns, goals, wishes, needs, and experiences.
  • Medication: Educate the patient and family on their medications. Providers should ensure that patients and their families understand the medications they are taking, the side effects they can have, and the risk versus reward of taking them at the end of life.
  • Mentation:  Understand mentation. This knowledge helps to lower the risk of changes in memory or mood.
  • Mobility. Maximize mobility.  Ensure the patient has as much mobility and independence as possible, which could mean the strength and balance to safely move around their house.

Tillery Compassionate Care Named as Prestigious 2024 Hospice CAHPS Honors Award Recipient

 

 May 22, 2024, Albemarle, NC— Tillery Compassionate Care has been named a 2024 Hospice CAHPS Honors Award recipient by HEALTHCAREfirst, a leading provider of CAHPS and bereavement survey programs for home health and hospice agencies. This prestigious annual review recognizes agencies that continuously go above and beyond in providing high-quality care for patients and compassionate caregiver experiences. It acknowledges the highest-performing agencies by analyzing the performance of the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey satisfaction and quality measures.

To be considered for this award, the hospice agency must have scored above the HEALTHCAREfirst National Performance Score on the Hospice CAHPS Willingness to Recommend question. If that criteria is met, performance is evaluated utilizing a set of 23 additional quality indicator measures. These scores are then compared on a question-by-question basis to a national performance score calculated from all partnering hospices contained in HEALTHCAREfirst’s Hospice CAHPS database.

Albemarle, NC (May 14, 2024) — Community Health Accreditation Partner, Inc., (CHAP) announced today that Tillery Compassionate Care has been awarded CHAP Accreditation under the CHAP Hospice Standards of Excellence.

CHAP Accreditation demonstrates that Tillery Compassionate Care meets the industry’s highest nationally recognized standards. The rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources, and long-term viability. Simply stated, adherence to CHAP’s standards leads to better quality care.

Tillery Compassionate Care Benefits from Long Partnership with the Old North State Club at Uwharrie Point

May 7, 2024 (ALBEMARLE, NC) – The women of the Old North State Club hosted a dinner recently to benefit Tillery Compassionate Care.  About 180 people attended the event held on Saturday, April 27, 2024, at the Old North State Club. A cocktail reception was held at the clubhouse before going to member homes for dinner. The event marked 21 years of support for the nonprofit with a whopping donation of more than $47,000 for 2024. Overall, the group has raised more than $410,000 for Tillery Compassionate Care throughout the past 21 years.

“Hospice is one of the few charitable organizations that has the potential to touch all our families at some point, and we are blessed to have such a quality hospice organization serving Stanly County and the greater Uwharrie region,” said Kerra French, Chairperson of the 2024 Old North State Hospice Committee. “The Old North State community is proud to continue our 21-year partnership with Tillery Compassionate Care and help support these critical services.”

 Tillery Compassionate Care Announces Camp Forget-Me-Not

 

March 16, 2024 (ALBEMARLE, NC) – Tillery Compassionate Care announces the date of their annual free grief camp for children and their families who have experienced a loss. This day camp will be held on Saturday, May 18 at Camp Joshua in Albemarle. Camp Forget-Me-Not is open to any grieving child in the community ages 5-18 along with their families.

We want to thank the  Stanly County United Way, who helps to make this event possible at no charge to the families,” said Chief Executive Officer Lori Thayer. “We encourage anyone in the community to attend if they have grieving children in the family, even if the death was not hospice related.”

The camp offers a safe and supportive environment for grieving children and their families with therapeutic experience through fun activities such as fishing, horseback riding, crafts, a scavenger hunt for a memory bear, and a butterfly release.

“Our Camp Forget-Me-Not is a day for children and their families to honor and remember their loved one. When children have the opportunity to speak and express their grief creatively, healing occurs in wonderful and unexpected ways,” said Bereavement Coordinator Michaele Conners, MA, LCMHC.  This day is for the entire family, to share memories together and have fun in a beautiful country environment. The adults often state they, along with their camper, receive so much from the camp and feel better in their grief process. We look forward each year to holding this amazing camp for our community. “

Tillery Compassionate Care Recognized as a 5-Star Hospice

February 26, 2024 (ALBEMARLE, NC) – Tillery Compassionate Care has been officially named a 5-star hospice by the Centers for Medicare/Medicaid Services (CMS).  Tillery Compassionate Care is the only hospice in Stanly and Montgomery counties to achieve a 5-star rating and one among only 14 hospices in the state of North Carolina to earn all 5 stars on the 5-star scale.

“What an honor to have CMS recognize the care that Tillery Compassionate Care has provided to our community again this year,” said Chief Executive Officer (CEO) Lori Thayer. “The 5-star rating is a direct reflection of our team’s love, care, and compassion demonstrated every day to our patients, families, and community.

The star rating system is based on results from a national survey that asks a family member or friend of a hospice patient about their hospice care experience. The rating system is intended to make it easier for consumers to use the quality measures to compare hospice agencies. More stars mean better quality.

Tillery Compassionate Care outperformed the national and state averages on each of the measures used to calculate the star rating. The publicly reported hospice survey measures include questions on the following categories:

  • Communication with family
  • Getting timely help
  • Treating patient with respect
  • Emotional and spiritual support
  • Help for pain and symptoms
  • Training family to care for patient
  • Rating of the hospice
  • Willingness to recommend the hospice

Tillery Compassionate Care awarded CHAP Accreditation

Albemarle, NC (May 14, 2024) — Community Health Accreditation Partner, Inc., (CHAP) announced today that Tillery Compassionate Care has been awarded CHAP Accreditation under the CHAP Hospice Standards of Excellence.

CHAP Accreditation demonstrates that Tillery Compassionate Care meets the industry’s highest nationally recognized standards. The rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources, and long-term viability. Simply stated, adherence to CHAP’s standards leads to better quality care.

“By achieving CHAP Accreditation, Tillery Compassionate Care has shown a commitment to excellence,” said Nathan DeGodt, CHAP President and CEO. “We are excited to begin our partnership with Tillery Compassionate Care by offering support in its commitment to providing quality care and continuous improvement.”

CHAP is an independent, not-for-profit, accrediting body for community-based health care organizations. Created in 1965, CHAP was the first to recognize the need and value for accreditation in community-based care. CHAP is the oldest national, community-based accrediting body with more than 9,000 agencies currently accredited nationwide. Through “deeming authority” granted by the Centers for Medicare and Medicaid Services (CMS), CHAP has the regulatory authority to survey agencies providing home health, hospice and home medical equipment services, to determine if they meet the Medicare Conditions of Participation and CMS Quality Standards. CHAP’s purpose is to define and advance the highest standards of community-based care.

Tillery Compassionate Care Benefits from Long Partnership with the Old North State Club at Uwharrie Point

May 7, 2024 (ALBEMARLE, NC) – The women of the Old North State Club hosted a dinner recently to benefit Tillery Compassionate Care.  About 180 people attended the event held on Saturday, April 27, 2024, at the Old North State Club. A cocktail reception was held at the clubhouse before going to member homes for dinner. The event marked 21 years of support for the nonprofit with a whopping donation of more than $47,000 for 2024. Overall, the group has raised more than $410,000 for Tillery Compassionate Care throughout the past 21 years.

“Hospice is one of the few charitable organizations that has the potential to touch all our families at some point, and we are blessed to have such a quality hospice organization serving Stanly County and the greater Uwharrie region,” said Kerra French, Chairperson of the 2024 Old North State Hospice Committee. “The Old North State community is proud to continue our 21-year partnership with Tillery Compassionate Care and help support these critical services.”

April 23, 2024 (ALBEMARLE, NC) – Tillery Compassionate Care recently held a banquet to celebrate and recognize the agency’s volunteers. Laynie Friend, guitarist & singer, provided entertainment for the volunteers and their guests.

“Volunteers are an essential extension of the TCC team.  Their kind words, warm smiles, and caring hearts are all things we should take time to celebrate every day,” said Chief Executive Officer Lori Thayer.  “We are grateful for our volunteers’ support and compassionate care! “

 Tillery Compassionate Care Announces Camp Forget-Me-Not

 

March 16, 2024 (ALBEMARLE, NC) – Tillery Compassionate Care announces the date of their annual free grief camp for children and their families who have experienced a loss. This day camp will be held on Saturday, May 18 at Camp Joshua in Albemarle. Camp Forget-Me-Not is open to any grieving child in the community ages 5-18 along with their families.

We want to thank the  Stanly County United Way, who helps to make this event possible at no charge to the families,” said Chief Executive Officer Lori Thayer. “We encourage anyone in the community to attend if they have grieving children in the family, even if the death was not hospice related.”

The camp offers a safe and supportive environment for grieving children and their families with therapeutic experience through fun activities such as fishing, horseback riding, crafts, a scavenger hunt for a memory bear, and a butterfly release.

“Our Camp Forget-Me-Not is a day for children and their families to honor and remember their loved one. When children have the opportunity to speak and express their grief creatively, healing occurs in wonderful and unexpected ways,” said Bereavement Coordinator Michaele Conners, MA, LCMHC.  This day is for the entire family, to share memories together and have fun in a beautiful country environment. The adults often state they, along with their camper, receive so much from the camp and feel better in their grief process. We look forward each year to holding this amazing camp for our community. “

Tillery Compassionate Care Recognized as a 5-Star Hospice

February 26, 2024 (ALBEMARLE, NC) – Tillery Compassionate Care has been officially named a 5-star hospice by the Centers for Medicare/Medicaid Services (CMS).  Tillery Compassionate Care is the only hospice in Stanly and Montgomery counties to achieve a 5-star rating and one among only 14 hospices in the state of North Carolina to earn all 5 stars on the 5-star scale.

“What an honor to have CMS recognize the care that Tillery Compassionate Care has provided to our community again this year,” said Chief Executive Officer (CEO) Lori Thayer. “The 5-star rating is a direct reflection of our team’s love, care, and compassion demonstrated every day to our patients, families, and community.

The star rating system is based on results from a national survey that asks a family member or friend of a hospice patient about their hospice care experience. The rating system is intended to make it easier for consumers to use the quality measures to compare hospice agencies. More stars mean better quality.

Tillery Compassionate Care outperformed the national and state averages on each of the measures used to calculate the star rating. The publicly reported hospice survey measures include questions on the following categories:

  • Communication with family
  • Getting timely help
  • Treating patient with respect
  • Emotional and spiritual support
  • Help for pain and symptoms
  • Training family to care for patient
  • Rating of the hospice
  • Willingness to recommend the hospice
