Hospice Aides

We are hiring full-time, part-time, and PRN (as needed) Hospice Aides to provide excellent, professional personal care to seriously ill patients. Hospice aide services augment care given by caregivers in a patient’s residence. The hospice aide functions under the direction, instruction, and supervision of the Registered Nurse (RN). Hospice Aides must document visit and tasks completion within electronic medical record and communicate patients’ current health status and needs to the RN. Candidates must be a Certified Nursing Assistant I or II; must have reliable transportation to and from work assignments. Medical Records Specialist

At Tillery Compassionate Care, we celebrate life in everything we do.  As the area’s only non-profit hospice organization, we are committed to excellence, compassion and dignity for our patients, families, and team members alike. 

We are hiring a Part-Time Medical Records Specialist, responsible for compiling, organizing, processing, and maintaining patient medical records within the hospice and care program to ensure records are current.

Responsibilities include answering and forwarding incoming patient/MD office telephone calls to nursing staff.  Ensures timely processing and completion of physician and clinical documentation, including but not limited to orders, certifications, recertifications, face to face visits, evaluation of terminal status and new patient visits. Creates Interdisciplinary group (IDG) meetings in EMR; generates and distributes IDG reports. Requests or obtains needed medical records for new admissions via a Health Information Exchange (CareConnect or NCHIE); performs follow up on outstanding items. Submits deaths to CAHPS vendor monthly. Reviews medical records to appropriately assign ICD-10 codes for all new admissions. Other duties: Participate in ongoing education and training offered by the agency. Comply with company policies, procedures, guidelines, and Mission Statement. Build and maintain key relationships within the community.  Other duties as assigned. You will work alongside our clinical and operational team members to ensure the provision of high-quality hospice services.

Minimum Qualifications: An associate degree in a related field, or combination of the completion of a MOA program and 2 to 3 years’ experience in responsible clerical work. High School diploma required.
