On Call-Rotating Schedule

Schedule: Full-time with rotating on-call schedule shared with another RN

We are seeking a compassionate, experienced, and highly skilled Registered Nurse to join our hospice team in a full-time after-hours, on-call position. This role is part of a rotating schedule shared with a partner RN and is critical to providing high-quality care and support to patients and families during evenings, nights, weekends, and holidays.

Key Responsibilities:

  • Respond promptly to after-hours calls from patients, families, caregivers, and referral partners
  • Triage and assess patient needs using sound clinical judgment and critical thinking skills
  • Prioritize and schedule visits based on urgency and patient needs
  • Conduct patient visits, including hospice admissions, in private homes, skilled nursing centers, and assisted living communities
  • Provide symptom management guidance and self-care interventions
  • Deliver compassionate and clear communication while actively listening and knowing when to gather more information—or when to simply provide reassurance
  • Identify and coordinate appropriate resources to support patients and caregivers

Qualifications:

  • Active, unrestricted RN license through the North Carolina Board of Nursing (NCBON)
  • Minimum 1–2 years of acute care experience required, including emergency services nursing
  • Hospice or home care experiences are strongly preferred
  • Demonstrated ability to work independently and exercise sound clinical judgment
  • Strong communication and interpersonal skills
  • Reliable transportation and flexibility to travel within the service area
