Who is eligible for hospice care?
To qualify for hospice care you must:
- Have a serious, life-threatening illness with a prognosis of six months or less to live.
- Agree to seek only comfort care, not treatment.
- Consent to hospice care. This means you will no longer seek to cure the disease
- Anyone can make a referral to hospice. This includes:
- Doctors
- Neighbors
- Family
- Friends
- You can even refer yourself to hospice.
What services are included in hospice care?
Hospice provides a wide scope of services aimed at providing peace, tranquility and dignity to a patient’s final months of life. These services include:
- Medical care from a team of experts trained in end of life care
- Spiritual care
- Patient and family support and counseling
- Medication management
- Assistance with daily activities
- Respite care for the family
- Bereavement counseling and support is provided to the family for up to 13 months or longer, if needed, after the death of their loved one.
- All medications related to the terminal diagnosis
- Medical supplies and appliances related to the terminal illness.
- Patient and family education (i.e. the team teaches the family caregivers how to provide care).
- Sort-term inpatient care, including respite care.
- Other services as required, including: physical, occupational, dietary and speech-language therapy.
How is hospice care provided?
This is provided by a multidisciplinary team that covers all aspects of comfort care, from administering medication to providing spiritual support. This means that your loved one is cared for by various professionals, and each one serves a specific role. This team includes:
- Doctors
- Nurses
- Chaplains
- Social workers
- Chaplains
- Volunteers
Is hospice care covered by insurance?
In general, hospice care is covered by private insurance, Medicare and Medicaid. However, we urge you to speak with your insurance representatives to get the most recent, up-to-date coverage details.
